Can it be caused from not washing your brand new mold prior to using it?

I know all the common causes of DOS, and have seen a few myself and eliminated the issues from my process, but on two occasions now I've poured cold process embeds and had them turn on me, every surface that touched the mold itself, with a mold that was brand new, and nothing else being different from the rest of the batch or curing process.

So why would you not wash a new mold before using it?

to be entirely honest I grabbed something because I had too much batter for my usual mold

It was just a curious thing I'd noticed not SOP for me. I (obviously) didn't actually use them for anything, I had them set aside with my larger scraps
