New soap recipe fail

Not sure what went wrong with this recipe. Ran it through soap calc. Hot process but never firmed up properly in mold. Any ideas?
Avocado oil 2 oz
Castor oil 1 oz
Coconut oil 9 oz
Olive oil 20 oz

Lye 4.5 oz
Water 12.5 oz

Here are a few questions. How old is your lye? Did you use sodium hydroxide? Did you weigh your ingredients or use liquid measurements? Just fishing for a clue as to why your soap did not firm up. I ran it through SoapCalc too and came up with the same weight for lye and water.
